FY26 DPHHS Community Integrated Health Pilot Site Grant Application

This grant opportunity is offered through Montana’s Rural Health Transformation Program (RHTP). The goal of this program is to ensure that every resident has affordable access to high-quality, high value care regardless of geography. The State’s RHTP plan directly addresses the root causes of rural health disparities through five integrated initiatives. Initiative three aims to modernize care delivery through several sub-initiatives. One goal of Initiative 3.2 is to deploy funding to Community Integrated Health Pilot Sites for FTE, equipment, supplies, travel and other approved expenses. Awards provided through this grant opportunity must be consistent with the state’s CMS approved RHTP plan.

This grant opportunity is open to emergency medical service agencies located in and/or serving rural areas(s) in Montana. Eligibility for this grant is based on a sites ability to administratively manage a Community Integrated Health (CIH) Program and obtain oversight from a medical director. Priority consideration will be given to agencies currently operating a CIH Program in rural or frontier areas and exhibiting the need for additional funding to remain operational or expand services. Agencies initiating a CIH program with an EMT/AEMT/Paramedic seeking CIH endorsement may be considered for this grant award. DPHHS may require proof of enrollment in a state approved CIH course prior to award. 

Under the grant agreement sites will be required to report data to the state, provide regular program updates, financial reports, etc. Sites will also receive support and TA from the state as programs are implemented or expanded.

Agencies awarded equipment must purchase equipment through a DPHHS approved vendor or group purchasing organization. Equipment, vehicles, technology, and supplies purchased with grant funds must be used for the approved CIH scope of work and eligible rural or frontier service area, maintained in accordance with manufacturer and agency requirements, recorded in the recipient’s property records, and made available for DPHHS monitoring. All equipment purchased must follow Uniform Guidance 2 CFR 200.313.
